For each week, fill in the date and time for all of your blog posts in the space provided for the appropriate assignment. Replace "<your blog(s) date(s) and time(s) here>" with the date(s) and time(s). For multiple listings, just put a semi-colon between each, letting the line wrap if you need to. If your post was made to the class home page, instead of your blog, please add “home page” after the date and time.
Include only those blog posts which meet the minimum word requirement. Do not list any comment postings. Do not include in-class writing assignment blogs. If you posted an assignment late and you and I discussed it, explain again why the assignment is late by adding an addendum to your Self-Evaluation.
After you complete this checkout sheet and save a copy, include it with your Self-Evaluation when emailing the instructor. Copy and paste this checkout sheet into the body of the email and attach your file.
